PHOTO & VIDEO RELEASE
My child and I are participants in the Music Togetherprograms offered by Mommy, Music & Me, inc. I hereby give permission to Mommy, Music & Me, inc. to take photographs and video recordings of my child, or me, or both of us together, and use them in printed or video formats for advertising and educational purposes. These mediums include, but are not restricted to brochures, ads, internet/website, etc.. The photographs and video recordings may be edited in your sole discretion, and I expressly release Mommy, Music & Me, Inc. and Music Together^®®, your agents, employees and assigns from all claims which I have or may have for invasion of privacy, defamation, or any other cause of action arising out of exhibition or distribution of any materials in which my child and/or I appear. I also agree that we may be identified by our first names and general place of residence. I further give permission to Mommy, Music & Me, Inc. to use any quotes that I may provide in writing regarding classes and Music Together® in all mediums in perpetuity.

I represent and warrant that I am the parent or guardian of the child whose name is taking the class  and that we shall both be bound by this agreement if I check the box.
